Trella Health Acquires Repisodic Hospital Discharge Automation Platform
Repisodic’s system was designed to streamline hospital discharges and connect patients with high-quality post-acute-care providers.

Trella Health has acquired Repisodic, a Philadelphia, Pennsylvania-based hospital discharge automation platform.

In an Aug. 6 news announcement, Trella Health said Repisodic’s system enables “intelligent, patient-centered care transitions.”

“Health systems are under growing pressure to lower inpatient length of stay, improve discharge adherence, and reduce readmissions — all while managing complex patient needs and shrinking margins,” said Scott Tapp, CEO of Trella Health. “Too many patients fall through the cracks during the most critical transition in their recovery — the move from hospital to post-acute care — and it’s costing lives, dollars and trust.”

The announcement added that the acquisition “marks a significant milestone in Trella’s mission to improve patient outcomes by combining intelligent workflows with actionable insights.” Repisodic’s technology “improves the hospital discharge process by delivering an EHR-integrated automation platform that helps leading health systems streamline care transitions and accelerate discharges. Health systems using the platform have reduced inpatient length of stay and increased referrals to high-quality post-acute providers.”

“Hospital discharge remains one of the most challenging and disconnected points in the patient journey, with many organizations still relying on paper lists, faxes, and verbal handoffs that create avoidable inefficiencies,” said Tyler Rardin, general manager, Trella Health. “With Repisodic, we’re expanding our data infrastructure further upstream — aligning insights, workflows and decisions from discharge through post-acute recovery to deliver better outcomes for patients and their families.”

“We started Repisodic to make it easier for patients and families to navigate post-acute care at hospital discharge,” said Mike Cwalinski, CEO, who co-founded Repisodic with Ryan Miller. “Joining Trella allows us to scale that mission and drive even greater impact across the care continuum.”

Cwalinski noted a case example of Repisodic working with a top-20 health system, saying that it enabled “connecting more than 1,400 post-acute providers with upstream access to patients and case managers at the point of discharge. Our health system footprint has grown almost 200% in the last 18 months, and we expect that pace to accelerate significantly alongside Trella.”

Trella Health, based in Atlanta, Georgia, now supports more than 50 health systems and more than 1,000 post-acute care organizations throughout the country.
