For those wishing to attend in person, the forum will be held at the Hubert H. Humphrey Building at 200 Independence Avenue S.W., in Washington, DC. In-person attendees must RSVP no later than 2 p.m. Eastern Time on May 11.
To RSVP, attendees should email their name, organization and telephone number to [email protected] with “Home Health” in the subject line. Upon entry into the building, they will be required to present to Security a government-issued photo identification, preferably a valid driver’s license.
CMS says TTY relay services are available for the hearing impaired wishing to attend the call, by dialing 7-1-1 or 1-800-855-2880. For Internet Relay services visit http://www.consumer.att.com/relay/which/index.html. A relay
