Cardinal Health Launches ContinuCare Pathway for Home Delivery of Diabetes Supplies
Publix Super Markets has signed onto the program.

January 20, 2026 by Laurie Watanabe

Cardinal Health (NYSE: CAH) has introduced a new way for patients with diabetes to receive diabetes supplies, including continuous glucose monitors.

The company described its new ContinuCare Pathway as a “unique pharmacy-to-supplier referral pathway program designed to help retail pharmacies seamlessly refer patients to Cardinal Health at-Home Solutions for the rapid fulfillment and direct-to-patient home delivery of diabetes supplies covered under the medical benefit,” Cardinal Health said in a Jan. 13 announcement.

While the program originally started at Advanced Diabetes Supply (ADS) on a smaller scale, it’s grown considerably since ADS was acquired by Cardinal Health in 2025. “Specifically, ADS has leveraged Cardinal Health’s robust access to brand-name home health supplies to triple its diabetes product offerings through the program with plans to expand to new product categories, including urology and ostomy,” Cardinal Health said.

In fact, the launch of ContinuCare Pathway was accompanied by the news that Publix Super Markets “has enrolled its entire pharmacy network in ContinuCare Pathway,” Cardinal Health noted. Publix has nearly 1,400 pharmacies in its stores in eight southeastern states.

“Providing diabetic supplies to patients covered under a medical benefit can create administrative burden on a pharmacy,” said Rob Schlissberg, president of Cardinal Health at-Home Solutions, said in the announcement. “ContinuCare Pathway helps pharmacies and patients navigate complex insurance requirements so they can get back to what matters most: prioritizing care.”

The new ContinuCare Pathway is available to any pharmacy in the United States, “from independent retail locations to retail, mass-merchandising and grocery chains,” Cardinal Health said. “The program can fulfill orders for patients covered under Medicare Part B, and most commercial medical and Medicare Advantage plans.”

ADS verifies patient coverage, then delivers the diabetes supplies to patients. Thanks to its fulfillment process, Cardinal Health said its at-Home Solutions business can ship packages across the country, “with most deliveries typically arriving in under two days of shipment.”

“ContinuCare Pathway helps ensure patients get uninterrupted access to their medically necessary devices and supplies,” said Michael Carmody, the vice president who heads the referral pathway program. “Designed specifically to fit neatly within pharmacy workflows to support continuity of care, patients get the right supplies when and where they need them — while pharmacies feel confident about referring their patients to a supplier that holds care, timeliness and compassion above all else.”
