TwelveStone Health Partners, a provider of post-acute, chronic care medication services, and VirtueRN, a post-acute healthcare technology company, will jointly develop a software platform aiming to streamline workflows for partners in post-acute care.
The effort aims to address the fragmented market for HME, infusion and medication services that is impacting referrals, partners and patients. Objectives for the project include streamlining workflow processes and increasing communication across the post-acute care continuum.

A statement from TwelveStone Health Partners said the company believe VirtueRN has a “head start” having creatied its own platform, and that will facilitate getting to market more rapidly and reduce overall project investment.
